Are you passionate about improving the lives of people with mental health issues, learning disabilities or substance misuse within the criminal justice system?
We are looking for two dedicated , motivated and proactive nurses or registered practitioners to work as part of our Street Triage Team. The team is established within Durham Constabulary but will be aligned to the wider crisis services already provided by Tees, Esk and Wear Valleys NHS Foundation Trust within the locality.
Main duties of the job
The team will work in partnership with Durham Constabulary to identify people with mental health needs and support them into appropriate services, creating effective partnerships and tackling stigma. We are looking for practitioners who can promote a culture of evidence based practice and provide excellent, responsive person centred care.

- Professional qualification related to MH nursing/social worker/OT
- Significant experience working with people with complex mental ill health, behavioural and social issues, in a community setting
- Worked within an acute/urgent care setting
- Assessed individuals in crisis situations
