We have a vacancy for a qualified practitioner in our Older Adult CMHT in Grantham. This would suit an experienced registered mental health nurse/social worker/OT; however we would be happy to receive and consider applications from a variety of qualified healthcare practitioners including registered general nurses, paramedics etc. We would also encourage a Band 5 practitioner to consider applying for the post and we will provide the structure and training to enable the successful candidate to grow professionally to meet the competencies of a Band 6 role. The Team provide a service across Monday to Friday 9-5, offering assessment, diagnosis, and treatment for patients of all ages presenting with memory difficulties and for older adults experiencing mental health difficulties within the Grantham and Sleaford area. Training and support for this will be provided if this has not previously been part of your job role.

The Team consists of a Consultant Psychiatrist, a Specialist Doctor, Senior Mental Health Practitioners, Associate Practitioners, and the Dementia Support Service. The Older Adult Division encompasses CMHT’s, inpatient wards and Home Treatment Teams as an alternative to hospital admission. As a division we work closely together on a joined-up patient pathway to ensure transitions between services are smooth and aid patient recovery.

The successful applicant will be providing high quality person-centred care to patients, families, and carers. You will act as care co-ordinator/Lead HCP, undertaking comprehensive evidence-based assessments, supporting patients to work towards recovery goals, build self-resilience and promote independent living. You will be caring for and coordinating a caseload of patients who are on both the mental health and the memory assessment pathway. You will also be working alongside Neighbourhood Teams, Adult Social Care, GP surgeries and other partner organisations to ensure the best outcomes for patients and carers.

Grantham Adult and Older Adult CMHT’s are actively involved in the Community Transformation roll out. Local Integrated Place Based Teams are being developed to support those patients with serious mental illness who do not need to be in receipt of secondary services. The Transformation Pathway is developing a step up, step down approach to ensure a smooth passage for patients through services at differing times of need, with a “no wrong door” philosophy.

The post holder will also have the ability to travel independently in the community to deliver services.
