A wonderful opportunity has opened up for an aspiring RMN to join our brand-new community home treatment team at the heart of Brent, where you will be afforded the time and space to engage in specialised training and development opportunities to help you become a more enhanced community practitioner. Working within a new service allows you to work with exciting and open-minded practitioners who are looking to create new ways of working, welcome new ideas and support one-another to set the standard. This is an opportunity to be part of a new initiative in Brent to support older people in crisis in the community.

Based on the outskirt rural areas of London, Brent is surrounded by beautiful parks and greenery where our staff can take advantage of the more peaceful areas. With buses taking you right outside our office and free parking in the surrounding areas, travelling to work has never been easier!

Main duties of the job

You will be expected to participate in the assessment and intervention offered by the older adult home treatment team. We are looking for an RMN who is highly motivated with excellent communication and clinical skills. Community Services are becoming fast paced and rapidly changing to support the 'Out of Hospital Care' strategies, you will therefore need to demonstrate resilience in the ability to adapt to the changing landscape and opportunities as the service develops.

To help you achieve the above our developmental role will also offer you;
  • Specialised HTT training including; how to undertake an effective assessment, ACE assessments, Duty/Triage/Referrals training
  • Opportunities to shadow our neighbouring services including CMHTs and the memory clinic allowing you to see the full circle of treatment for our patients
  • Mentoring and coaching sessions with team leaders supporting you with your development and growth into the role
  • Clear objectives and pathway to work towards Band 6 role

  • The Older Adult Home Treatment Team has a dedicated workforce including Consultant, speciality Doctor, a Team Manager, Band 7 Clinical Leads, Band 6 and 5 RMNs supported by Band 4 clinical and administrative staff.
  • The team works together, removing any hierarchical forms of working where all staff are empowered to use their skills and abilities
  • Shift patterns allowing more flexible working and a healthier work-life balance. We are a 7 day a week service working Monday-Friday 8am to 8pm and 9-5 on weekends.

Job description Job responsibilities

  • To be accountable for the older adult service users . To assess health and social care needs (including accommodation, finances, support in accessing education, training or employment) and negotiate care plans with service users and their carers, remaining objective when working with diagnostic
  • To prioritise time and workload, in a flexibly manner to meet community caseload responsibilities, participate in meetings and effectively develop personal skills knowledge and experience through supervision and
  • To undertake risk assessments, according to Trust
  • To ensure delivery of care plans in partnership with multi-disciplinary colleagues and a range of health and social care agencies, within the CPA
  • To engage with service users and carers assertively and offer care/interventions within the least restrictive setting, in normal community

  • To help clients gain fair and equal access to services, through choice and social inclusion and being sensitive to age, culture, ethnicity and gender, sexuality and disability

    Vaccination

    The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users.     • Demonstrate ability to assess, develop, implement and evaluate programmes of care for individuals experiencing acute mental distress.
    • Demonstrate Knowledge and skills in assessing and managing risk
    • Demonstrate ability to co-ordinate a team and it's day to day work
    • Demonstrate ability to use a range of therapeutic interventions with individuals in acute mental health crisis.
    • Demonstrate understanding of CPA and it's application
    • Demonstrate good communication skills, including listening, reflecting, empathy, facilitation, persuasion, motivation, negotiation, education and reassurance, even in challenging and sensitive situations.
    • Demonstrate ability to determine and reduce risk to self, clients and others.
    • Demonstrate understanding of the role of HTT and of the social model of care.
    • Demonstrate a commitment and the ability to work as part of a team.
    • Demonstrate knowledge of issues and developments within Mental Health and the National Health Service.
