To provide an integrated Adult Congenital Heart Disease level 2 service for patients in South and West Wales.

To work as part of a multi professional team, ensuring that patients with ACHD receive the highest standards of care and support from the time of diagnosis through the course of their disease.

Working closely in delivering the network priorities whilst supporting treatment pathways for patients with ACHD

Main duties of the job

This post is 12 months fixed term to cover maternity leave.

The ACHD CNS's currently cover the geographical area of South and West Wales. You will be given the opportunity to support our peripheral clinics in West Wales and support the inpatients/outpatients at our base in UHW.

This is an excellent opportunity as the successful candidate will take a lead in developing areas specific to this role eg surgery, intervention, heart failure, obstetrics, specialist services for young people transitioning into adult services, learning disability, psychological support, cardiac obstetric services and end of life care.

You will also be responsible for providing support in the form of outreach to the intermediate and localACHDcentres. You will act as an expert resource for the multidisciplinary team, providing specialist education in the community and of colleagues at all levels. You will practice autonomously and will be supported to develop advanced skills such as differential diagnosis, physical assessment and prescribing.

Excellent collaboration and communication will be essential through regular network meetings as well as national clinical meetings to ensure continuous service developments.

You should possess Leadership skills that will support initiating change whilst improving clinical practice and service development.

The ability to speak Welsh is desirable for this role Welsh and/or English speakers are equally welcome to apply.

  • MSc or equivalent level of knowledge and experience
