The Psychiatric Liaison Services at the Ablett unit, Denbighshire, North Wales offers an assessment service to the acute hospital population.

The team provide a 24/7 assessment service to the acute hospital with a team of nurse practitioners, team manager, consultant psychiatrist, specialty doctor and team secretary. Also working alongside the Adult team are-Older Peoples Liaison Nurses, Alcohol Liaison Nurses, specialist Occupational Therapist, pharmacist and healthcare support workers.

The opportunity has arisen to recruit another band 6 nurse practitioner to join the psychiatric liaison Team for12 months Maternity Cover

The post holder will provide comprehensive mental health assessments, referrals and follow-up arrangements.

Main duties of the job

The post holder will triage individuals accessing the ED for health and mental health issues risk assessment and prioritization of interventions and assessment to facilitate an appropriate timely response. This is of significant benefit to patients attending the emergency department by reducing waiting times, streamlining the transition through the department and improving follow-up appointments. To support this process, the post holder will have excellent knowledge of Primary and Secondary Mental Health and Substance Misuse Services as well as Community and Tertiary services.

The post requires a high level of ability to undertake psychosocial assessments, risk assessments and draw up safe and effective discharge and progression plans. This includes the use of assessment tools, crisis intervention, signposting to community resources, follow-up, short-term contact clinics, and education for individuals, families, staff and the public.

The post holder needs to be able to work well with-in the team but also be confident and competent to work independently, and will exercise an appropriate level of independent judgement when setting work priorities.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers will be given the same opportunity to apply.
