The role
An exciting opportunity has risen in South Hams and West Devon OPMH CMHT for a permanent post of 0.6 WTE Band 7 Advanced practitioner. Non-medical prescribing competency is essential.

The successful applicant would be joining an experienced team of Advanced Practitioners delivering high quality care to older adults with functional and organic mental illness and play and play an integral role in supporting the wider team working closely with the consultant.

About you
The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced person who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills and prescribing for patients within the service. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.

The post holder will work as a member of the multi professional team by providing an advanced clinical role to prevent un-necessary acute hospital admissions and where safe, appropriate and in the persons best interest to do so keep patients in their own homes for longer, work to identify and proactively prevent premature relapse and/or deterioration in a person’s condition hence where possible avoid admissions into long term care facilities.

The post holder will work as part of a multi professional team providing a clinical service that is sensitive to the needs of the population. The team will provide comprehensive assessment in order to establish diagnosis and creation of an appropriate person centred evidenced based treatment plan this may include the independent prescribing and regular review of treatment with a medication, working closely with the medicines optimisation team.

The Advanced Practitioners will contribute to the development and implementation of effective models of care to facilitate timely, clinician to clinician advice.
The location
Your base will be in Totnes, covering the South Hams and West Devon locality, however under the current COVID-19 conditions this may be subject to alternative arrangements/change for example home working.
