An exciting new opportunity has arisen to join our friendly department as part of a CrossSite Medicines Optimisation Pharmacy Technician. This is a new role, which would suit an experienced solution focused Medicine Management Technician who is happy to work autonomously in a patient facing role. This role will require regular travel between hospitals and community sites, travel expenses will be provided.

You will rotate between hospital wards and the community and will work independently to provide a seamless transfer of care from in-patient to community services. You will work closely with community mental health teams (CMHT) and Chief technicians to expand the role to ensure the safe, effective and economical use of medicines in the community.

Main duties of the job

Community Services
  • Involved in the co-ordination of the supply of Depot injections to Patients through Polar Speed.
  • Support the maintenance of the repeat prescriptions, to ensure they are in date and discrepancies are highlighted in a timely manner to avoid unnecessary waste or dose inaccuracies.
  • Review community mental health patients to ensure physical monitoring has taken place.
  • Review clozapine patients to ensure essential serum monitoring has taken place.
  • Participate in patient counselling in the community in conjunction with the Community Mental Health Teams.
  • Support the ESCA referrals.
  • Support the management of Clozapine Service in conjunction with local Community Mental Health Teams.
  • The post holder will be required to work independently within the Community environment

  • Ward Services
  • Complete medicine reconciliation.
  • Identify patients ready for discharge. Participate in patient counselling at ward level as necessary and provide follow up in community where necessary.
  • Support the Discharge Medicines Service.

  • Person Specification

    QUALIFICATIONS & TRAINING

    Essential

    • BTEC and NVQ3 (or equivalent) in Pharmaceutical Science
    • Nationally recognised Accredited Checking Technician Certificate
    • Registered as a Pharmacy Technician with the General Pharmaceutical Council


    Desirable

    • Nationally recognised accredited medicines management technician qualification.
    • Ability to produce reports using a variety of systems


    EXPERIENCE

    Essential

    • Working as a registered Pharmacy Technician in the UK
    • Hospital experience
    • Experienced medicine management technician


    SKILLS, KNOWLEDGE & ABILITIES

    Essential

    • Able to work as part of a team
    • Ability to work autonomously
    • Ability to know when to refer
    • Ability to travel on a regular basis and at short notice
    • Able to act on own initiative
    • Ability to communicate orally and in writing e.g. good telephone manner, excellent face to face communicator.
    • Patient counselling skills
    • Computer skills involving the use of email, word, excel, internet.
    • Able to prioritise own workload
    • Able to take responsibility for personal development and contribute to Service Development
    • Ability to work in a busy environment to deadlines.
    • Able to work Monday to Friday and at weekends/bank holidays if required. Be on a rota half a day a week in role of ACT.


    Desirable

    • Familiar with Ascribe/CPMS/SCR
