We are recruiting an experienced professional to lead our transfer of care and discharge hub across the Sheffield health and social care system.

The post will be hosted with the Sheffield Teaching Hospitals Foundation Trust and work on behalf of Sheffield Place for the people of Sheffield.

This is an exciting, newly created role, allowing the successful candidate to make a real difference within our system. You will be supported by a System Discharge Lead and programme manager as we continue to work on our transformational change programme alongside our business as usual supporting people to get to the right place, with right care at right time as quickly as possible. Sheffield is passionate about ensuring people and those who care for them have positive, safe and timely discharge or transfer of care experiences, that are focussed around what matters to them.

The successful candidate will be an experienced operational professional, with good working knowledge of hospital discharge and admission avoidance policy and best practice. They will demonstrate excellent communication skills, have relevant experience of service transformation and working across organisational boundaries.

Main duties of the job

You will lead a multi-disciplinary team made up of registered health and social care professionals, administration staff and others all involved in supporting people to return home after discharge from beds in the acute and community settings.

The postholder will provide operational leadership and coordination to enable transfers and support recovery in the community. The Hub is focussed around a Home First, and 'why not home, why not today' philosophy and the postholder will operationally embed and support this goal.

The post holder will have significant opportunity to shape the design of the service and its continuous improvement, and will be responsible for ensuring we have effective systems, processes, and data to enable as few delays as possible for people.

The postholder will support the operational implementation of the Hospital Discharge and Community Support Guidance.

This post is a system post and will lead this work alongside all system partners from the SheffieldIntegrated Care Board, Sheffield Teaching Hospitals NHS Foundation Trust (Integrated Community and Acute trust), Sheffield Health and Social Care Foundation Trust, Sheffield City Council and ensure engagement with all partners including the voluntary and community sector, primary care, and private care providers as well as health and social care organisations working together.

Person Specification

Education and Qualifications

Essential

  • Educated to Academic Level 7 (Masters degree or post graduate qualification plus additional expertise, evidence of specialist knowledge and experience)
  • Evidence of post graduate qualification and continuing professional development


Experience

Essential

  • Experience of working at a senior level in specialist area (Nursing, Social Work, AHP's)
  • Previous experience of implementing a Discharge to Assess or Home First service
  • Proven ability to lead, teach and act as a role model at senior level
  • Risk management and clinical governance

Essential

  • Driving licence or access to transport to attend external events/meetings/sites
  • Excellent IT skills and confident with Microsoft packages
