This post is being advertised to address a temporary need for 23 months however the Trust has made the commitment to offer the successful applicant a permanent contract with the Trust. In the event that the need for this post ends, a suitable alternative post will be offered. Applicants should be aware that alternatives may require some flexibility on role and/or location.
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via our recruitment fairs.
Main duties of the job
We are pleased to be able to offer an opportunity for a band 5 Nurse to join our well established memory service in York and Selby. The post holder will work as part of a multi-disciplinary team to provide assessment and primary treatment for people experiencing brain change. They will be supported by other Nursing colleagues, an Advanced Practitioner, Psychologists and Consultant Psychiatrists within the role.
The post holder will undertake sessions both within clinical environments and individual service users own homes. As such, it is essential the post holder is capable of independent travel.
We feel this is an exciting opportunity for someone to develop specialist skills, supporting people who experience brain change.
Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis.
About us
There have been significant changes within the service as a result of Covid-19 restrictions. This has afforded the service with an exciting opportunity to review and refresh the Memory Service model. The successful applicant will be encouraged to be an active part of these developments and to implement service improvements over the course of their secondment. They will contribute to the ongoing evaluation of the changes to the service model.

The post holder will lead on the assessment and treatment where clinically indicated; for people seeking clarity about changes they are experiencing. We have an MDT led service in which all clinicians are involved in the formulation of a diagnosis and the resulting interventions being offered. Should medication be clinically indicated the post holder will be responsible for the titration and physical health monitoring of those on their caseload until appropriate transfer to primary care is completed.The service is a diagnostic and medication led team with post diagnosis support offer by third sector partners. As a result the post holder will be responsible for the onward referral of people where appropriate.

Person Specification Qualifications Essential
- RNMH, current professional registration with NMC.
- Evidence of Continuing Professional Development.
- Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
- Post-graduate qualification in a relevant area
- Post-qualification experience of working in mental health services for older people
- Previous experience of working within an MDT
- Previous experience of working in a memory service
- Previous experience of multi-agency working
- Clinical Risk Assessment and Management and its application in practice.
- Understanding of Safeguarding and its application in practice.
- Clinical Governance and Clinical Supervision.
- Manage own time and prioritise effectively
- Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team.
- Recent evidence of undertaking basic physical health screens
