The Homeless Service is an excellent award-winning service (MBE award, two-time Winner of Lammy Award, Regional Winner of Health and Social Care Award for Improving Health and Reducing Inequalities, Winner of GSTT Care Award, Winner of Ab Fab Award) and friendly team delivering services to those experiencing homelessness. You will work in non-NHS sites such as hostels, day centres and street nursing when required and will be expected to travel to the Gracefield Gardens office when needed and we offer hybrid working and flexible working opportunities.
We are currently recruiting to a Band 7 Nurse practitioner sexual health trained (Adult) post 12 months fixed term contract or secondment. The post holder will primarily provide a sexual health care and lead a sexual heath outreach project within the Homeless service

Main duties of the job
Leading the sexual health project- attending meetings and holding a case load of clients.
Running sexual health in-reach clinics at hostels for the homeless
Sexual health treatment - administration of PGD medications
Referral into main stream services
Safe guarding referrals - high risk client group.
Vaccine administration
Advocating for individuals
Liaising with accommodation providers, Local authority and stakeholders.
Participating in the running and development of the inclusion service service.
Leading on key roles within the homeless team and participating, as a senior member of the team in the leadership and development of the whole team.
Supporting the service leads in up holding the 7 pillars of governance

The Band 7 nurse will work closely with the other band 7's to offer leadership and support to the whole team.
To work autonomously, managing a caseload of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care.
Be responsible for assessing and recognising emergencies in the specialty.
To interpret information and take appropriate action and to lead others to do the same.
Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
Direct, support, encourage and advise clinical teams, by the provision of specialist knowledge and advice in the care of patients within the area.
Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care.
Support and enable patients and carers to make informed decisions relating to their treatment and management.
Promote high quality clinical care, and assist in the development of the specialist service, through the use of evidence based practice and clinical standards.

- BSc/1st degree/equivalent qualifications or experiance
- Evidence of comprehensive professional development in sexual health and contraception. This will include a sexual health course and a recognised contraception qualification such as the DFSRH

- Have or working towards a Masters degree

- Evidence of recent updating in Health inclusion - providing service to marginalised groups.
