Are you an experienced Mental Health Practitioner looking to work autonomously, in a forward thinking, fast-paced environment?

Whether you are a Registered Nurse, Occupational Therapist or Social Worker; this is the ideal challenge for someone who is looking to make a difference to the local communities of Lowestoft.

Lowestoft is a thriving seaside town and has received significant investment in the last few years; so, you can expect plenty of development happening in the area. You'll be working across 7 different practices alongside an experienced lead mental health practitioner, as well as another Band 6. We're a collaborative team and work closely with all those who are based at the practices.

Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.

The Lowestoft teams are passionate about developing their mental health provision; they're proactive and enthusiastic and need a forward-thinking, driven mental health practitioner to help them achieve their aspirations.

Main duties of the job

As a PCN MH practitioner, you will work across the interface between Primary Care and Secondary Mental Health Services undertaking the planning and delivery of direct patient care.

The ideal candidate will have an interest and experience in a mental health setting; you'll have good organisational skills as you will be managing a caseload, as well as offering support and advice to other health care professionals. You will be an autonomous practitioner, making clinical judgements, identifying alternative courses of action, managing and providing high-quality care.

We're committed to training, continuous professional development and ensuring all members of the team can access resources to help them to develop and progress. When you join us, you can expect to be supported from the start; you'll spend time with our community teams learning about the different roles they have, as well as shadowing other members of the PCN.

These roles, although new have been piloted in our Early Adopter sites for the past year so the workload is defined, manageable and appropriate to allow for patient centred care and other activities

Qualifications
Essential
  • Nursing degree or equivalent or Dip OT, Dip SW, Clinical Psychology Grade A, or equivalent.
  • Mentorship qualification or ENB 998. Willingness to train to become Practice Supervisor or Assessor

Desirable
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice
  • ILM Certificate in management or equivalent.

Experience
Essential
  • Experience in working in team delivering person centred treatment packages
  • Demonstrate effective risk assessment and risk management experience

  • Experience in supporting service development

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people. We are a large organisation covering a wide geographical area divided into ‘Care Groups'. There are also Care Groups supporting children, family and young people’s services, and specialist services, including Forensic mental health services.
