The Wigan Specialist Community Mental Health Team (SCMHT) are looking to welcome two Community Mental Health Nurses (Band 5) to the team to support our SCMHT team.

These post follows on from our first successful round of recruitment as part of our Community Transformation Plan and implementation of the streaming model of care in the SCMHT. The post holders will support the nursing streams within the SCMHT and will be supported by a Band 7 Stream Lead, a Band 6 Senior Nurse Practitioner, and Support Staff, as well as the wider MDT.

Additional clinical support and supervision will be offered by the SCMHT leadership team.

While experience would be desirable, we will consider supporting preceptors at Band 5 level or staff working on inpatient units who would like to transition to a community mental health service.

This post is ideal for those who enjoy care co-ordination and working with patients on standard care who are accessing secondary care mental health services. The role varies from day to day and offers opportunity for flexibility and positive challenge as no two days are ever the same.

This post is a full-time position (37.5 hours) and the working hours will be 8am to 8pm 7 days a week. We also encourage applications from staff wanting to work part time hours. Flexible working can be discussed. The post is based at Claire House in Wigan. The post holder will be expected to travel across the Borough to attend satellite clinics across Wigan and Leigh.

Main duties of the job

The Specialist Community Mental Health Team (SCMHT) provide assessment, care, support, treatment, intervention, advice, guidance and liaison for adults aged 18 years and over, with severe and enduring mental health problems with high complexity of needs.

The post holder will lead in the organisation and delivery of nursing interventions to patients with secondary care needs allocated to a nursing stream.

This includes:

- management of care delivery to an identified caseload of patients

- responding to patients, families, carers and professionals in a timely manner when contacting the service

- support with the completion of 72 hour follow ups and 7 day follow ups for patients discharged from inpatient services into the care of the SCMHT

- support with the facilitation of inpatient admissions and/or referrals to the Home Based Treatment Team

There will be a requirement to travel as part of this role as the post holder will also facilitate the administrator of depot injections for patients open to the team who are unable to attend the clinics.

The post holder will be well supported as staff wellbeing and development is paramount - full supernumerary status will be provided to support induction to the team for the first 4 weeks in post. Your skills and development opportunities will be supported as part of your induction process and reviewed regularly during line management supervision and an annual appraisal.

Person Specification

Qualifications

Essential

  • Registered Mental Health Nurse - diploma or degree in Mental Health Nursing, Social Worker or Occupational Theraist


Experience

Essential

  • Demonstrable post qualifying experience


Desirable

  • Experience of working within a community setting


Knowledge

Essential

  • Evidence of continued professional development
  • Experience of receiving clinical supervision
  • Demonstrates ability to use initiative to deal with situations which may occur
  • Ability to work collaboratively with colleagues and those from other disciplines
  • Have good verbal and written communication skills
