The North Community Mental Health Team in Trafford is an integrated health and social care service and strives to provide quality recovery-based support to people with complex and or enduring mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the CMHT operational procedure.

Main duties of the job

Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the CMHT operational procedure.
You will be responsible for the care coordination of a caseload, assessing service users mental health and risk and managing your caseload according to the CPA policy.
This includes respecting the individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
