Oxleas would like to invite applications from suitably qualified candidates for a permanent post of Specialty Doctor to join our friendly and forward-looking Primary Care Plus team.

We are seeking to recruit a Specialty Doctor for our Primary Care Plus Team at Market Street Health Centre. The post holder will work alongside another full time consultant. The service is a locality based pathway of care which is the direct link between primary and secondary care services and focuses on tele-triage, providing direct advice and support to GPs and directing service users to the pathway that will specifically meet their needs.

The directorate actively supports continuing professional development of its staff - including any additional training needed to help achieve personal and professional goals. We have a strong commitment to patient safety and positive patient experience. Candidates who have other special interests will be encouraged to develop them and a job planning meeting will be offered within 3 months of taking up appointment.

Main duties of the job

  • Support to the consultants and the PCP team in developing and maintaining the caseloads of all patients ensuring that Core Assessments, Risk Assessments and Care Plans are available in RiO case records.
  • Provision of reviews including Mental Health Act where necessary to the PCP team.
  • Development of comprehensive case-note summaries, comprehensive multi-axial assessment processes and care planning in line with NICE guidelines.
  • Attend PCP team meetings.
  • To ensure the involvement of families and carers, wherever possible, with the patient's consent in treatment planning.
  • To support the consultants in preparing any necessary reports, and attending Mental Health Review Tribunals if applicable
  • Providing clinical cover for the consultants, when she/he is away on leave, with supervision by another consultant.
  • Daily case reviews of patients seen during Routine Tele Triage (RTT).
  • Working closely with local GPs and attending regular GP meetings.

Job description Job responsibilities

Primary Care Plus (PCP) provides a specialist mental health liaison service to primary care. The service has four core aims:
  • Assessments and referrals management
  • Relapse prevention and shared care
  • Health Promotion
  • Education, training and consultation
  • Assessments & referrals management

  • The PCP Team provides a single point of access for all referrals to Oxleas Secondary Care Mental Health Services and general adult psychiatrist, from primary care (GPs and IAPT).

    Urgent referrals will be telephone triaged the same day and if needed will receive a same day face to face assessment from PCP or our crisis service. Routine referrals will be triaged within 2 weeks.

    New referrals will then be directed to the appropriate treatment pathway including psychosis, ADAPT and Early Intervention.

    If PCP receives referrals for other directorates these will be sign-posted accordingly i.e. Older Adults and Memory services to have their full assessment and treatment. All tele-triage and face-to-face assessments are complaint with the Care Act 2015.
  • Relapse Prevention & Shared Care

  • For people who have previously been under Oxleas services, PCP aims to support primary care to maintain people in remission or with stable optimally controlled symptoms and develop good self-management skills.

    PCP has a link worker for every GP practice, who will be accessible and provide liaison support between primary and secondary care.

    PCP prevents relapses by quickly re-engaging people who drop out of treatment. PCP provides early access to specialist mental health practitioners as soon as early warning signs of relapse occur.

    PCP psychiatrists will support GPs to manage complex medication regime sometimes over the telephone and provide advice and oversight for issues such as off-license prescribing, monitoring, and input into the updating of shared care prescribing guidelines.

    The CMHTs offer psychological consultations for people who are not willing or able to access treatment pathways (for example for personality disorder), and help GPs formulate a care plan for that person.
  • Health Promotion

  • PCP promotes physical health by offering brief interventions, signposting and linking in with other services.

    PCP provides information, advice and guidance as required by the Care Act (2015) including signposting, and support to access resources; e.g. financial advice services, accommodation, leisure and educational resources.
  • Education Training & Consultation

  • Consultant Psychiatrists are available by phone to GPs during their primary care consultations with patients, offering real time advice and guidance via Consultant Connect.

    PCP will be in the forefront of the Primary Care Network developments, providing education and training to GPs about mental health.

    PCP, in conjunction with the locality CMHTs, provides regular teaching to borough wide GPs and other members of the primary care teams.
