We are looking for a motivated and skilled nurse, occupational therapist or social worker to join us as a Band 6 Primary Care Mental Health Practitioner. You will be working with a colleague to deliver mental health services to patients in primary care. This post, in conjunction with a full-time colleague, covers the GP Practices across the Dacorum locality (Hemel Hempstead, Berkhamsted and Tring) and its principles are in line with the NHSE Five Year Forward View for Mental Health. This recognises that by placing specialist mental health staff in GP practices who promote recovery-focused early assessment and treatment, we will help prevent deterioration and ensure good outcomes for service users. This role offers the opportunity to work from home at least some of the time as a number of service users continue to voice a preference for telephone triage/intervention.

The post holder will join a team of Band 6 mental health practitioners who are based within the primary care setting (GP practices). You will also be able to work with colleagues in newly recruited Additional Roles Reimbursement Scheme Mental Health Practitioners (a partnership role between primary care and HPFT).

The post holder will provide:
  • specialist advice to GPs and other primary care colleagues
  • timely advice, support and interventions to service users (and where relevant their carers)
  • assist primary care practitioners in the transfer of service users from secondary care to primary care or primary care to secondary care.

This post will also provide the opportunity to work alongside various professional staff groups including GPs, practice nurses, pharmacists, paramedics, associate physicians, receptionists, community navigators and the wider community provision.

We are seeking applicants who
  • have extensive knowledge and experience of working with individuals with mental health needs
  • possess advanced assessment and formulation skills
  • are keen to develop their clinical skill base.
  • are confident, flexible and dynamic in their approach to their work with a positive outlook to working within a busy primary care environment.
  • able to work autonomously whilst also being a good team player.

In return for this you would be offered continued training, and an opportunity to contribute to service development whilst working within a friendly and supportive team. The post holder will receive clinical support and supervision from operational managers and a Consultant Psychiatrist.
