Service/Ward/Department Summary:Tameside Adult Acute WardsWardare based at Tameside General Hospital.Saxon Suite provides assessment, care and treatment for individuals with a variety of acute mental health needs from a team of professionals including nursing staff, medical staff, occupational therapists, a psychologist, a dietician and a pharmacist. We provide a range of personalised therapeutic interventions which focus on individual strengths and promote recovery.

Role Summary:The post holder will be expected to undertake all aspects of the role identified in the job description and meet the requirements of the person specification. The role will be to deliver all aspects of care to the service user group on the wards and to undertake the responsibilities required of a qualified nurse.

Main duties of the job

Key Responsibilities of the Post:o To assess, plan, implement & evaluate care of service users defined as requiring in-patient care. To take on the role of bleep holder to acute services. To liaise with key stakeholders to ensure seamless care delivery across acute serviceso The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of initiatives within the acute service.o Providing evidence based interventions to clients and relatives to aid recovery from current mental illness.o The post holder is expected to supervise junior staff and act as a mentor and assessor for all learner bands.o The post holder will engage in clinical supervision and promote this service to others within the unit.o The post holder is expected to show initiative and develop and implement change ideas within the acute Service.

Job SummaryThe postholder will be responsible for the assessment of needs, planning,implementation and evaluation of programmes of care for patients admitted toTameside & Glossop adult inpatient wards.The postholder will ensure the highest possible standard of nursing care inaccordance with the Policies and Procedures of Pennine Care NHS FoundationTrust, the Mental Health Act and Community Care Legislation.The postholder will participate in clinical supervision and have an active role in thedevelopment of the Nursing Team, including participation in the EducationalProgrammes of students and others.

Main Duties and ResponsibilitiesProfessional DutiesTo be actively involved in the development of team nursing on the Adult In-patientward, assuming the role and responsibility of a named nurse by assessing,planning and implementing programmes of nursing care for individual patientswithin your allocated team.In collaboration with patients develop individual care plans and refer onto otheragencies as needed.Involve families / carers, were appropriate, throughout the admission pathway inthe development of treatment packages, being mindful of their educational needsand level of support.To evaluate and modify nursing care in accordance with the Care ProgrammeApproach and facilitate the developing discharge plans.To actively participate in multi-disciplinary meetings and contribute to clinicaldecisions regarding clients in your care.To manage caseloads effectively, making the best use of time and resources.To ensure that any unresolved issues for clients and their relatives/carers areidentified and referred to the appropriate personnel.To keep accurate and timely professional records of nursing care, ensuringconfidentiality is maintained.To assist in the development of service standards and clinical audit.To participate in development and implementation of new initiatives, incorporatingresearched based practices.To participate in clinical supervision.

Education and DevelopmentTo participate in programmes of personal development based on agreedobjectives.To participate in the assessment of students allocated to the acute Mental HealthService and in conjunction with other disciplines ensuring their developmentneeds are addressed.To be aware of research based practices with particular reference to acutemental health care.To act as a mentor and / or assessor to students and to undertake mentorshiptraining.To be involved in preceptorship and mentorship of colleaguesMaintain a professional portfolio of personal developments and assume an activerole in self-development and continuing education.

ManagementTo report to ward manager / deputy ward manager as appropriateYou will be responsible for administrative duties on the ward as delegated by theWard Manager.In consultation with the Ward Manager establish priorities in work load and organise working day to make effective use of resources.To deal with complaints from clients and relatives in accordance with Pennine Care NHS Foundation Trust Complaints Procedure.To participate in ward meetings and Task Groups as requested.Participate in supervision, advice and support for team members.This job description is not exhaustive, but is intended to give an overall picture of therole. Person Specification Education / Qualifications Essential
  • Must be first level registered nurse, RMN or RN in Mental Health.
  • Must have a good understanding of both innumeracy and language skills.
Desirable
  • Qualification in psychosocial interventions an advantage
  • Completed Preparation for Mentorship
Knowledge Essential
  • Knowledge base in terms of this client group.
Experience Desirable
  • Current experience of adult acute inpatient wards
Skills and Abilities Essential
  • Be able to perform required duties of the post.
Desirable
  • Experience of working within a psychosocial model of care delivery
Work Related Circumstances Essential
  • Must be flexible and able to work on an internal rota basis across nights and days
  • Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
  • Willing to carry out all duties and responsibilities of the post in accordance with the Trust's Equal Opportunities and Equality and Diversity policies
  • Appointments to regulated and controlled activities require an enhanced DBS disclosure.
