This is a new permanent role created to help us deliver our plans to transform our community mental health services, as part of the NHS Long Term Plan and working within our Primary Care Networks. The MH ACP role will work within GP practices supporting people who identify with mental illness.

Our Mental Health transformation plans are supported by £25m in funding and will mean the roll-out over the next three years of multi-disciplinary teams of experts working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community. Our aim is to support the whole person as they live with, or recover from, mental illness and to offer them support with other challenges they may face, such as debt, relationship problems, housing, education or training. We will also focus resources on reaching those who currently face obstacles in getting the help they need.

Main duties of the job

The Mental Health Advanced Clinical Practitioner (MHACP) will use highly specialist knowledge and skills to provide healthcare autonomously to people within their sphere of practice, acting as the Mental Health expert. The MH ACP is accountable for independent clinical assessment, diagnosis and treatment of people. In addition to this, the MH ACP is also responsible for appropriately referring people to relevant specialties for any necessary investigations.

The MH ACP will practice at an advanced level demonstrating in depth knowledge and competence in aspects which may include acute medicine, community services and mental health; encompassing enhanced assessment, critical thinking, intervention planning and delivery and clinical management skills that are evidence based. The MH ACP will deliver high-quality person-centered trauma informed care.

The MH ACP, based in the PCN's, will bridge with Barnet Mental Health Community Core teams and will lead in the creation of robust communication and referral pathways, being a conduit for primary care PCNs and Barnet Community Mental Health Teams.
