As we roll out our exciting plans for the community mental health framework DPT and its PCNs partners are looking to employ a Band 7 Specialist Community Practitioner to join the Crediton Community Mental Health Team, and to work within one of the two PCNs which cover the Crediton and Okehampton areas of Mid Devon. The successful candidate will be joining a dynamic, motivated and skilled team and through your leadership and clinical knowledge, you will be a key part of the clinical leadership in your PCN. You will be working as a member of the multi professional team providing an enhanced clinical role based within the PCN, and working collaboratively with the assessment practitioner, GP, other members of the Multi Agency Team and the Core Mental Health Team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required
- Are you an experienced registered mental health practitioner looking to challenge yourself in an exciting new community role as a Specialist Community Practitioner based within the GP Surgery
You will be working closely with the consultant psychiatrist and team manager to help with initial and ongoing prescribing, general medication and clinical advice for complex clinical cases.

You will be an experienced registered mental health professional and ideally a non-medical prescriber / therapy experience as well with excellent communication skills and a confident manner with patients and colleagues.
The post holder will demonstrate safe, competent clinical decision-making and have high levels of clinical competence in assessment and diagnostic skills. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. About us
