Supporting patients who are acutely unwell in the community, and providing the best level of care to them whilst remaining in their own homes. We provide support for individuals who are experiencing an acute episode in the community, as well supporting people who have been discharged from inpatient wards. We provide a menu of interventions, to ensure the highest level of care and support is delivered.

You will need experience working in mental health care as a band 5 qualified practitioner. Experience of leadership and recovery focused practice is required.
Working over 7 days week, covering a 24 hour shift pattern. The team cover the West Kent area including Maidstone, Tunbridge Wells and Sevenoaks.
Main duties of the job
- To work within the acute setting as a valued member of the multi-disciplinary team providing and contributing to high quality nursing care for service users with severe mental health problems.
- Be responsible for the risk assessment, planning, implementation and evaluation of emotional and physical care needs for service users, and be fully aware of the needs of service users over a 24 hour period.
- Monitor, report and maintain a healthy, safe and secure workplace by knowing and taking account of: Health and Safety at Work Act (1974), Reporting of injuries, diseases and dangerous occurrences regulations (1985), Control of substance Hazardous to Health (1985), European H&S Regulations (1992)-Numbers 1-6, Food Act (1996), Adhere to Trust policies and local service protocols.

- Assess, plan, implement and evaluate care planning and risk assessment with service user on admission and at appropriate intervals
- Ensure all interventions are documented clearly and comprehensively.
- Communicate and collaborate with other members of the multi-disciplinary team to effectively utilise resources to help meet identified service users needs
- Ensure the correct administering and documentation of medicines in accordance with Trust Policy, legal requirements and NMC standards for medicines management.
- Contribute to the prevention and management of abusive, aggressive and challenging behaviour in line with least Restrictive practice
- Participate in supervision, annual appraisal and competency assessments in order to reflect on and develop own practice.
- Maintain PIN via the revalidation process
- Promote & ensure the good reputation of the trust. To act as a positive role model maintaining professionalism at all times and to demonstrate a positive attitude to service users, their families, visitor and others.
- Report and document any incident and escalate in line with trust policy and actively participate in reviewing and learning from workplace related incidents.
- Be aware of and work within Safeguarding Adults and Children policies, be able to recognise the signs of abuse and report and document concerns to Team Manager and MDT.
- Competently and confidently, write, present and discuss information regarding service users care and treatment.
- Ensure that the service users rights and dignity are maintained at all times.
- Understand how to escalate concerns regards care and treatment and how the whistleblowing policy complements this.
- Willing and helpful to support and provide cover to other teams at short notice under direction of senior staff.
- Ensure high levels of hygiene and infection control systems are maintained.
