The team consists of a Consultant in Palliative Medicine, a doctor in training, a Clinical Psychologist, a Head of Service, a team of 8 Specialist Palliative Care Nurses, a specialist Occupational Therapist, a specialist Physiotherapist and a Technical Instructor. In addition two Palliative Support sisters and a team of palliative health care assistants deliver hands on personal care to patients in the last weeks of life. We work closely with GP's, District Nurses and other local services such as Myton Hospice.

In order to support the end of life pathway and ongoing Out of Hospital work streams we have identified a need for four band 5 nurses with palliative skills to support new referrals into the service and co-ordinate a rapid response to patients in their own homes and care homes. The service currently works between the hours of 8.00am to 10.00pm with a requirement to work over 7 days per week.

About us

The service received an overall rating of Good by the CQC and an Outstanding for care and compassion of which we are rightly proud. We have also been recognised both locally and nationally by receiving awards for the services we provide.

Coventry and Warwickshire Partnership Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry, Warwickshire and Solihull.
