JOB TITLE: Band 7 Senior Mental Health Practitioner (Based in South Lincoln Healthcare Primary Care Network)
DEPARTMENT: Integrated Place Based Team (IPBT)
LOCATION/BASE ADDRESS: Lincoln South
BAND AND SALARY: Band 7
NUMBER OF HOURS: 1.0 wte (37.5 hours per week)
TEMP/PERM (END DATE IF FIXED TERM): Permanent contract

The Role:
Whilst based within the locality Integrated Place Based Team in Lincoln you will be assigned to the South Lincoln Healthcare PCN to provide mental health leadership and implement clinical processes. In addition to undertaking mental health assessments, you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care.

Whilst a full Job and Person specification is attached to this advert, key elements of the role will include (but not be limited to):

• Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support service users with highly complex mental health needs/serious mental illness.
• Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the primary care network (multiple GP practices)
• Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
• Managing a caseload of highly complex service users, working closely with the locality secondary mental health service, to maintain care plans and avoid escalation : It is expected that this will equate to 50% of the working week.
• To perform the duties of a non-medical prescriber, ensuring that appropriate medication is prescribed, monitored and administered in line with current best practice and the Trust Medicines Management Policy.

The Successful Candidate will:
• Have the qualifications and experience as highlighted in the Person Specification.
• Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
• Preferably have some experience of working closely with or within primary care.
• Demonstrate adaptability, excellent communication skills and self-motivation.
• Be able to demonstrate appreciation of service users needs and a track record of improving care

The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.
