An exciting opportunity has arisen for an enthusiastic and dynamic nurse to join and lead the Heart Failure Specialist Nurse team within the Cardiology department at South Tees Hospitals.

Main duties of the job

The post holder will have clinical, administrative, leadership, policy and service development responsibilities.

The post holder will develop and maintain a fully comprehensive nurse facilitated Heart Failure service within the tertiary centre and in primary care settings, incorporating new developments and technologies as they arise. They will also be responsible for leadership and line management of the Heart Failure specialist nurse team.

The post holder will be an expert clinical practitioner who will utilise advanced clinical assessment skills to ensure patients receive high standards of evidence-based care intervention and support from diagnosis through to end of life care, to promote a better quality of life and reduce morbidity and mortality.

After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.

Person Specification



  • A first degree - with nursing focus preferred
  • MSc in relevant area with nursing focus or within specialist field preferred.
  • Registered Nurse


  • Attendance at the leadership and development and improvement programme (4.5 days)



  • Extensive experience and knowledge of working within heart failure services at a tertiary and community level.
  • A record of experience of innovation in clinical practice.
  • Experience in coordination of running of multiple MDT meetings.
