Oxford Health and Buckinghamshire’s’ Primary Care Networks (PCN) are leading the implementation of the Community Mental Health Framework to improve patients experience and plug the gap between Primary and Secondary Care Mental Health Services. These emerging roles will empower patients aged 18+ years to access specialised mental health support locally, within their GP Surgeries.

The post holder is integral to accomplishing ease of access to mental health care for Buckinghamshire’s population at primary care level in an efficient and timely manner. The successful applicant(s) will facilitate improved opportunities for joint working with Buckinghamshire’s voluntary care sector, whilst contributing to the leadership and development of national mental health agendas across their matched Primary Care Network (PCN). As part of the Primary Care Networks Multi-Disciplinary Team (MDT) your role will provide mental health triage, assessment and advice to patients and your colleagues. You will work to facilitate early intervention for patients, to enable recovery within the community, whilst recognising the value in your skills to ensure prompt access to secondary care specialists as appropriate.

This role provides the opportunity for continuing professional development (CPD), with scope to influence service innovation and development. This role is for practitioner(s) who pride themselves on delivering a high-quality holistic approach for patients needing mental health support. The following Primary Care Networks are eager for a Specialist Mental Health Practitioner and currently have a vacancy: Dashwood PCN, Arc PCN, BMW PCN, Chesham and Little Chalfont PCN, Cygnet PCN and South Bucks PCN. Whilst each PCN holds an overall population, your role will be to work within your contracted hours to meet the needs of a proportion of these patients. Within each PCN there are bespoke teams of Additional Role Reimbursement Scheme (ARRS) practitioners who will act as your Multi-Disciplinary Team, working collaboratively to improve health inequalities.

Successful candidate(s) on appointment as Specialist Mental Health Practitioner roles will receive up to four weeks formal bespoke induction, organised collaboratively between Oxford Health and PCN, with capacity for continued informal induction at candidate’s consideration. You will be based within the GP surgeries on a day-to-day basis, with the opportunity for hybrid working. Candidates will be employed by Oxford Health and will receive three layers of supervision, with additional supervisors available for complex areas of practice and an establishedpeer support network for theseroles across our integrated care system, which successful candidates will receive protected time to attend. About the Job
  • To provide a lead role in supporting the PCN in triaging mental health concerns raised by on the day patient requests, highly providing specialist advice and support within GP surgeries
  • To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams
  • To offer brief interventions as needed eg problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice and prescribing if a registered NMP. Identifying care act eligibility and referring appropriately for assessments, carers assessments or safeguarding concerns.
  • Working as a member of the primary Care MDT and taking responsibility for the development and implementation of specialist mental health training to the PCN.
  • To provide a navigation role to help GP’s make appropriate referrals to the relevant community and secondary care services – bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.
