A fabulous opportunity has arisen for a Lead Nurse within the Derby City Older Adults Community Mental Health Team.
The team is busy and dynamic, assessing and treating people over the age of 65 with mental health conditions, and a diagnosis of, or suspected dementia.
The post holder will work with a fantastic team, comprising of Nurses, Occupational Therapists, Psychiatrists, Doctors, Nursing Assistants, Psychologists and Team Administrators.
The team covers a wide geographical area. The position is based in the South Derbyshire team.
Potential team members can expect monthly team meetings, registered staff meetings, regular supervision, in-house and external training, alongside the day-to-day support of a nurturing and friendly team.
Main duties of the job
The role of Lead Nurse/Mental Health Practitioner can be challenging and varied with the management of a complex caseload, completion of initial assessments, undertaking the role of duty worker, attending CST meetings and working with other professionals to provide the best outcomes for our patients.
The successful candidate will need excellent communication skills, the ability to complete thorough assessments, including safety assessments, be able to produce person-centred care plans with a living well/recovery focus and work within the CPA framework and Community Mental Health Framework.

- Carry a complex caseload
- Assess service users with a functional/ organic illness and formulating care plans in the diverse area on Derby City
- Undertake duty triage referrals, communicate with other disciplines and signpost to the appropriate services

- Extensive knowledge of the principles of Safety planning, care planning and risk assessment
- Knowledge and application of assessments and interventions and outcome measures, relevant to client group
- Experience of working in a CMHT and/or working with Older Adults in a mental health setting
