A fabulous opportunity has arisen for a Lead Nurse within the Derby City Older Adults Community Mental Health Team.

The team is busy and dynamic, assessing and treating people over the age of 65 with mental health conditions, and a diagnosis of, or suspected dementia.

The post holder will work with a fantastic team, comprising of Nurses, Occupational Therapists, Psychiatrists, Doctors, Nursing Assistants, Psychologists and Team Administrators.

The team covers a wide geographical area. The position is based in the South Derbyshire team.

Potential team members can expect monthly team meetings, registered staff meetings, regular supervision, in-house and external training, alongside the day-to-day support of a nurturing and friendly team.

Main duties of the job

The role of Lead Nurse/Mental Health Practitioner can be challenging and varied with the management of a complex caseload, completion of initial assessments, undertaking the role of duty worker, attending CST meetings and working with other professionals to provide the best outcomes for our patients.

The successful candidate will need excellent communication skills, the ability to complete thorough assessments, including safety assessments, be able to produce person-centred care plans with a living well/recovery focus and work within the CPA framework and Community Mental Health Framework.

South and Dales OA CMHT is a friendly and dynamic team of experienced staff.

The successful candidate will receive clinical and management supervision on a regular basis, and will be supported to develop as a leader personally and professionally.
  • Carry a complex caseload
  • Assess service users with a functional/ organic illness and formulating care plans in the diverse area on Derby City
  • Undertake duty triage referrals, communicate with other disciplines and signpost to the appropriate services
  • Supervise staff
  • Be actively involved with the care homes project and CST meetings
  • Work within a multi-disciplinary setting

Qualifications

Essential

  • Registered Mental Health Nurse
  • Educator/Mentor Qualification


Skills

Essential

  • Evidence of service receiver and carer involvement activity
  • Understanding team dynamics and promoting a positive working culture to support team cohesion.
  • Ability to build effective working relationships internally and externally to the organisation
  • Ability to work autonomously and set own priorities
  • Computer literate
  • Ability to carry and manage own caseload


Knowledge/Experience

Essential

  • Post-registration experience as a Mental Health Nurse
  • Experience of working with people who have severe and enduring needs
  • Experience of supervising staff and students
  • Working knowledge of the principles of CPA (Care Programme Approach)/Community Mental Health Framework where appropriate
  • Extensive knowledge of the principles of Safety planning, care planning and risk assessment
  • Knowledge and application of assessments and interventions and outcome measures, relevant to client group
  • Experience of working in a CMHT and/or working with Older Adults in a mental health setting


Desirable

  • Experience in Older Adult Community Mental Health
