This is an exciting position which is open to Registered General Nurses or Registered Mental Health Nurse's which will focus on the physical wellbeing of our client's with mental health needs. The post holder will be responsible for the leading on the physical health care agenda within the Trafford living well Service under the supervision of the Team Manager.

The post holder will ensure the delivery of all physical health care and trust key performance indicators. They will act as a specialist resource and advisor to colleagues in the Trafford living well and other VCFSE colleagues within the neighbourhoods. The post holder will work across community settings, including clinic bases and home visits.

The post holder will be required to work, for the majority of time, as an autonomous clinical practitioner for patient groups across a range of clinical settings,. The post holder will be expected to forge strong links with the case manager's, Physical Health Support staff, Primary Care services particularly GP practices.

Main duties of the job

Assessment of Physical healthcare and developing management plans

Experience of completed Physical Health investigations and associated interventions such as completing the PHIT tools.

Experience of undertaking physical health observations including taking bloods and completing ECG's

Effective communication skills and experience in the management of others

Ability to act on own initiative and provide advice and support to nursing staff and to work closely with the Psychiatrist and the Senior Pharmacist in the team.

Ability to liaise and communicate with a range of internal and external personnel

Ability to use clinical information/IT systems

To implement change within a clinical setting

Education and Training

Essential

• Registered Nurse
• Evidence of post registration qualifications or training

Desirable

• Training in: venepuncture, cannualtion IV Therapy, ILS, Ecg reading and taking, reading and sign posting blood results
• Acute Chronic wound management, clinical examination skills, Mentorship in practice. Willingness to undertake the above

Experience

Essential

• Substantial experience of working as an autonomous clinical practitioner
• Experience of working with mental health patients in any health/social care setting
• Experience of working as part of a team in order to achieve a shared vision/goal

Desirable
