There is an opportunity for a newly qualified or experienced Mental Health Band 5 Nurse to join our OP CMHT at Carlton Court Hospital in Lowestoft. We are seeking a motivated and enthusiastic individual who has a passion for supporting older people with functional MH issues or have a dementia, both within their own homes or residential care settings covering Lowestoft and Waveney areas. Our experienced team will help and support you develop all the skills you need as you hold a small caseload and provide support to our senior clinicians, completing 117 reviews, assessments in care homes, supporting patients on depots and creating care plans as needed. We are a small team of Nurses, AHP's, APs and CSWs with two Psychiatric Consultants and support from a Clinical Psychologist. This post will support you to further develop your community nursing skills.

Main duties of the job

- Supporting care coordinators in case management

- Observation and information gathering about the service user

- Care planning / Risk Assessments / Diagnostic testing such as ACE 111

- Building rapports and positive working relationships patients and carers

- Attending team meetings and participating in reflective discussions.

- Undertaking 117 reviews / depot administration to community patients

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people. There are also Care Groups supporting children, family and young people’s services, and specialist services, including Forensic mental health services.
