An exciting opportunity has arisen for a suitably qualified and experienced professional to hold a Clinical Lead Practitioner role within the Hounslow East MINT Team, one of three MINT teams in the Borough of Hounslow. The teams have recently launched following a successful bid for additional resources and all are at an early stage in their formation

The Long-Term Plan, together with NHSE's Community Mental Health Framework, provides a historic opportunity to address gaps in the provision of Mental Health care within communities. Community teams will move away from siloed, hard-to-reach services towards joined-up care and whole population management approaches, establishing a revitalised purpose and identity for community mental health services. One of the LTP's key objectives is to develop new and integrated models of primary and community mental health care [which] will support adults and older adults with severe mental illnesses.

It supports the development of place based and personalised community mental health services wrapped around the Primary Care Networks (PCNs), and outlines how these developments will help to improving care for people with severe mental illnesses.

Main duties of the job

Support the team to offer enhanced access and provision to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and coexisting substance use.

In addition, the lead practitioner will ensure that the team work proactively to address social and racial disparities. Key to the success of the teams will be establishing and maintaining relationships within the local primary care networks as well as the Local communities in supporting the delivery of this vision.

Prior experience of working across a range of community mental health settings

Hold a registration for the correct governing body such as the Nursing and Midwifery Council, Social Work England or the Health & Care Professions Council

Knowledge of and experience in supervising the community psychiatric nurses in delivering a range of brief interventions to patients with mental health needs;

Good written and verbal communication skills and the ability to use these to strengthen local relationships with the local MINT Teams

Person Specification Qualifications Essential
  • oRegistered Nurse, (mental health) appropriate to the job role.
  • Evidence of post registration training in relevant service area
  • Completion of Mentorship Course/ENB equivalent/Practice Educators

  • oExperience of supervising staff and students
  • oAble to manage a caseload of people with complex mental health needs
