Community Mental Health team in Redcar as looking for a highly motivated Band 5 Community Nurse. This is an exciting time to join the community mental health teams with the transformation of our community services currently under way.
This is an exciting opportunity for new or experienced practitioners to work as part of a multi-disciplinary team in the Redcar & Cleveland locality. As a Community Psychiatric Nurse you will have a genuine interest in collaborative working with people of varying ages and varying needs, including service users, their carers and families.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team manager and the Advanced Practitioner.
Main duties of the job
As a core member of the team, you will also be responsible for your own caseload, and delivering interventions in the community. In line with transformation we are moving towards teams who offer the right intervention and the right time and therefore the work will be a blend of treatment pathways.
You will ensure that the service users' recovery goals are identified and central to their well-being and recovery plans, promoting social inclusion, independence and well-being. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
The team is embracing a number of changes to improve the quality and effectiveness of the services provided in the community to our patients and as a community psychiatric nurse you would be an essential element in facilitating this change.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team manager and the Advanced Practitioner. Clinical Responsibilities, Patient ContactMaintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision-making and are fully safeguarded.Acts, wherever applicable, in accordance with the Mental Health Act and associated policies and procedures.
Responsible for recognising the potential for or signs of patient harm, abuse or neglect, including poor clinical practice, reporting all such concerns and taking all reasonable steps to protect the patient. Responsible for identifying and reporting concerns regarding the safeguarding of children who may be at risk.Ensures the highest professional standards and attitudes towards the care of patients from all staff. Patients have a range of mental health conditions, may demonstrate behaviours that challenge and may have difficult family situations of relevance.Undertakes the initial assessment of new patients, under supervision, on a rota basis.Processes new referrals and has responsibility for own caseload with support from a senior community psychiatric nurse or team manager.Assesses patients needs and develops, implements and evaluates intervention plans as part of the multi-disciplinary team, with the involvement of the patient and where appropriate, their carer. Guidance is provided by a senior community psychiatric nurse or advanced practitioner as required, for more complex patients.Ensures intervention plans are based on current risk assessment, evidence based practice, critical thinking and whole system support requirements that take account of relevant physical, social, cultural, psychological, spiritual, genetic and environmental factors.Undertakes risk assessments in accordance with the Trusts Clinical Risk Assessment and Management Policy, devising and implementing actions and intervention plans which take the risk formulation fully into account.May be required to accompany and provide support to patients within a variety of community settings, dependent on current risk assessment. This may include social inclusion/community integration/transfer to and from appropriate areas e.g. acute hospitals, police stations etc.Responsible for the safe transport and administration of medicines in accordance with Trust policy, legal requirements and NMC guidance, where applicable. Administers intramuscular injections.Supports individual patients and their families/carers in the community, providing advice and liaising with other support services to co-ordinate individual support packages.Provides health education and advice on health promotion to patients and their carers.Responds appropriately to crisis situations, where necessary seeking advice and guidance from other relevant professionals and where appropriate being responsible for decision making and coordination in a crisis situation.
Carries out venepuncture as required, if trained.Demonstrates safe moving and handling of patients using equipment as required.Demonstrates safe breakaway techniques in the management of violence and aggression as required.Demonstrates safe moving and handling of patients using equipment as required.Where appropriate and with authorisation, acts as an advocate for patients/carers.May be required to attend Mental Health Act tribunals.Participates in the duty rota as required.

Person Specification Qualifications Essential
- RNMH, current professional registration with NMC.
- Evidence of Continuing Professional Development.
- MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards. Qualification must be achieved within agreed timescale.
- Working with people with mental ill health.
- Working in a community setting
- Evidenced based knowledge of best practice in caring for patients in the designated field.
- Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
- Awareness of psychological models of care and treatment.
- Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team.
- Work effectively as part of a multidisciplinary team.
- Capable of independent travel
