We are seeking a motivated, Highly Specialist Physiotherapist to join our community Learning Disability Healthcare Team. As a friendly and dynamic team we provide specialist services for adults with learning disabilities across Wandsworth.

You will need specialist clinical assessment skills and the ability to work with people who present with complex physical disability. You will be involved in 24 hour postural management, MDT working in dysphagia management, mobility assessments and falls prevention, as well as optimising exercise opportunities for adults with learning disabilities. Assessment and intervention will take place in a variety of community settings including clients' own homes, supported living facilities, registered care homes and community day services.

You will not only carry a varied caseload but will also provide clinical leadership to a small team of physiotherapists and assistants. You will be supported in this aspect of the role by the Clinical Team Leader.

The successful applicant will work closely with the multidisciplinary team comprising of community nursing, dieticians, occupational therapists, speech and language therapists, psychologists and psychiatrists. The post holder will also develop excellent working relationships with other NHS services and external agencies.

Main duties of the job

  • To work as an autonomous practitioner within the standards of professional practice laid down by the Charted Society of Physiotherapy (CSP) and Health and Care Professions Council (HCPC).
  • To use available evidence base and work in partnership with service users, carers, colleagues and other stakeholders.
  • To offer interventions that are person centred, respecting choice and values.
  • To use a problem solving approach, identifying and managing risk, and working in the most appropriate environment for service users and their carers ensuring equitable and accessible service provision.
  • To independently manage a caseload of service users with complex health and social care needs.
  • To provide professional and clinical leadership for the physiotherapy team
  • Ensure physiotherapy team meets agreed targets and objectives

Clinical
  • To work as an autonomous and often lone practitioner within the standards of professional practice laid down by the Charted Society of Physiotherapy (CSP) and Health and Care Professions Council (HCPC). Work is undertaken with a diverse, highly complex population of service users, many of whom have multiple pathologies or no formalised medical diagnosis.
  • To use available evidence base and work in partnership with service users, carers, colleagues and other stakeholders.

Professional
  • To maintain registration and comply with the HCPC Standards of Proficiency, Conduct, Performance and Ethics. To comply with the CSP Code of Ethics and Professional Conduct.
  • To maintain accurate and up to date electronic records, in line with Trust and local policies and to assist in regular audits to check compliance and act on findings to ensure improvement in standards.

Communication
  • To communicate in a polite, courteous and professional manner at all times.
  • To use a range of verbal and non-verbal communication approaches to communicate effectively with service users and carers. This will include service users who may have difficulties with understanding or communicating, for example non-verbal, dysphasic, depressed, hearing impaired, visually impaired or with limited use of English. Using creative, innovative strategies including multimedia tools will assist in minimising the communication barriers.

Education and Research
  • To educate service users, carers and colleagues in relation to an individuals needs and the effects of their disability, disease and/or injury on function, lifestyle and independence, the management of these and likely prognosis.

Management and Leadership
  • To provide professional and clinical leadership for the physiotherapy team
  • Demonstrate ability to lead change through people communicating the vision & rationale for change.


Person Specification Training and Qualifications Essential
  • Diploma/Bsc in Physiotherapy.
  • HCPC registered
  • Evidence of post registration training relevant to field
  • Leadership and /or Management training
Desirable
  • Member of relevant clinical interest group e.g. ACPPLD (learning disabilities)
  • Clinical Educators Training
Experience Essential
  • Minimum 3 years experience as a Band 6 physiotherapist in a variety of acute and community settings in NHS
  • Evidence of multi-disciplinary and multi-agency working.
  • Experience of carrying a caseload of service users with complex and diverse needs
  • Experience of the supervision of junior staff within a formal supervision scheme.
  • Experience of maintaining a written record of CPD
  • Evidence of working in a multi-cultural context
Desirable
  • Evidence of participation in service development.
  • Experience of working with people with learning disabilities or people with neurological conditions, as a Physiotherapist or student Physiotherapist
Knowledge and Skills Essential
  • Thorough and in depth knowledge of the complex physical health needs of people with learning disabilities.
  • Thorough and in depth knowledge of current national health and social care legislation and local policies relevant to people with learning disabilities and how these impact on the provision, development and delivery of the physiotherapy service.
  • Knowledge of impact of Clinical Governance in relation to practice.
  • Highly skilled in being able to communicate complex and sensitive information to service users and carers.
  • Highly skilled in being able to prioritise workload effectively and manage own caseload
Desirable
  • Knowledge of the role of Physiotherapy in dysphagia management
  • Knowledge of specialist equipment and postural management systems.
