An exciting opportunity has arisen for a Mental Health Practitioner (Band 5) to join the Spalding Community Mental Health Team. This post would suit a professional with a desire and commitment to meeting both the challenges and opportunities available in providing excellent rural mental health care. The role will require the successful candidate to provide care co-ordination to service users who are experiencing a severe and enduring mental health illness. They will be required to assess, plan and implement care for those on their caseload. The successful candidate will also be required to undertake initial recovery focused biopsychosocial assessments focusing on the services users personal goals and strengths and signpost appropriately. The service is delivered through a multidisciplinary team who provide comprehensive assessment and intervention. The team operates Monday – Friday between the hours of 9am-5pm providing care to those living within the town of Spalding and the surrounding area. A sound knowledge of Care Programme Approach, NICE Guidelines and the Mental Health Act is desirable.
