A great opportunity to join our Older Persons Mental Health Team for a Band 5 development post as a Community Mental health Practitioner within the Portsmouth Older Person Mental Health Team (OPMH).
It has been identified that the transition from being a practitioner on the ward to being a practitioner within the community setting can be challenging. This will involve working within the Adult Acute/PICU inpatient care setting and working in the older persons inpatient care setting. Alongside these placements there will be a structured training package which will enable the post holder to further develop their skills and knowledge, which will be transferrable to the community care setting.
The applicant would need to be a motivated, enthusiastic practitioner, who is able to facilitate skilled assessments and treatment interventions to the older adult.
You must be genuinely interested in working with older adults and provide compassionate care to patients and their families with knowledge of organic and functional illness with a good understanding of physical health problems.
You will play an important role in the team as we aspire to provide excellent care to our patients and develop our service in line with national and local guidance.
You will be expected to work within the wider MDT implementing and evaluating care and managing a caseload of patients. You will supervise junior staff and students within the team. This is a really exciting time to join our team as the Mental Health Community Services will be undergoing a transformation which the successful applicant will have the opportunity to participate in.
