Main duties of the job
To undertake the role of key worker/care co-ordinator providing assessment, implementation and evaluation of individuals' care. To ensure the care given is patient centred, multidisciplinary, collaborative and evidence based.
Interested applicants will need to demonstrate an understanding of the recovery approach and be committed to working with patients who have serious and enduring mental health problems with sometimes complex and challenging behaviour.Successful applicants will be given the opportunity to build on existing skills and learn new skills required to enable the successful rehabilitation and move on of patients to more independent settings.
