A new opportunity has arisen for an Advanced Nurse Practitioner, who is a NMP, within our Older Persons Community Mental Health Team (OP-CMHT) based at Bridport Community Hospital.

This team comprises of medical staff (Consultant and F2 Dr), Team Leader, RGN/RMNs, StR workers, and a psychologist. This multi-disciplinary team provides a comprehensive specialist mental health service for the Bridport sector of West Dorset (including Bridport, Lyme Regis, Beaminster and the surrounding areas) according to a defined group of primary health care teams.

The postholder will be the nominated CMHT lead for the Bridport Community Hospital wards mental health referrals( 2 x 18 bed step up/ step down- currently open as 1 x 18 bedded ward- Colmers). The service takes new referrals on those persons presenting with a mental disorder age 65 years and above, transitional patients with later life needs from the adults of working age service and persons of any age with a dementia.

The Team is based at the community hub in a shared office with the adults of working age Team. There is a suite of clinic rooms and the facilities are modern. They are co-located with social services, community rehabilitation team, physiotherapists, community matrons, frailty team and the hubs. They work closely with these other professionals- participating in twice/ week virtual reviews. Upstairs is a physical health ward ( Colmers) and minor surgical procedures suite. There is also a general outpatients department and a Minor Injuries Unit. The Hospital itself has its own parking and is set on the outskirts of Bridport which is a bustling market town with many independent shops and businesses.

The Team have a close working relationship with primary care colleagues. The post holder would be encouraged to participate in these links- through monthly Virtual Ward Rounds

The team itself comprises:
  • Band 7 Team Leader
  • Band 6 RMNs/ RGNs
  • Band 3 StR workers
  • Admin support within the CMHT admin pool
  • Psychology sessions
  • F/T Consultant psychiatrist
  • F2 Dr
  • Occasional ST6 Drs

The successful applicant will be responsible as part of the clinical team for providing mental health assessment, treatment and care planning for patients accepted by the Team and will work under the supervision of the Consultant psychiatrist.

There is an expectation that the post-holder will see patients in outpatient clinics, community hospitals or carry out home visits to see the patients in their own homes.

The ability to work closely and co-operatively with members of a multi-disciplinary team is an essential and integral part of the post. Attendance at a weekly team meeting is required. The presentation of assessments, management plans and case reviews at the weekly team meeting is an important part of the duties of this post.

The ANP will have supervision from the consultant in the OP-CMHT for Bridport. The post holder will be expected to identify a clinical supervisor. The postholder will participate in regular supervision (management and clinical).
