An exciting opportunity has arisen for a committed, competent and passionate senior nurse practitioner to join Warrington and Halton Mental Health Liaison Service Core 24 Team

The team provides timely assessments to individuals with mental health and psychological needs who present to Warrington General Hospital. Warrington and Halton Core 24 Liaison Service is a friendly and dedicated team made up of Senior Nurse Practitioners, Team Manager, Clinical Leads, Clinical Psychologists, Consultant Psychiatrists, Occupational Therapist and Admin Support.

While experience of working within mental health liaison services is preferable, we appreciate mental health nurses will have relevant transferrable skills in assessments and leadership and we welcome applications from candidates with other relevant experience.

Main duties of the job

The post holder will provide comprehensive assessments to service users utilising recognised assessment tools and in line with the organisations policy and procedures.

The post holder will work closely with stakeholders ensuring that service user need and risk are accurately communicated.

Job description Job responsibilities

CLINICAL

Participate in regular clinical and managerial supervision as per Trust policy.

Participate in caseload supervision to monitor standards of service user care and documentation.

Assist with the appropriate screening of referrals and provide information for referrers, service users and carers.

Provide short-term evidence based practices and fixed term interventions.

Support the delivery of education and self-help materials within a variety of locations.

Develop and maintain effective working partnerships with services in other statutory and non-statutory organisations.

Assist in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.

Ensure the clinical environment is conducive to promote service users health, safety and well-being, thereby preserving and respecting each services users personal dignity, privacy, religious and cultural beliefs.

Ensure record keeping is in accordance with professional and Trust standards.

Work in a professional and collaborative manner with the multidisciplinary Team.

Responsible for the assessment of needs & risks and the development, implementation and evaluation of programmes of care without supervision.

Non-Medical Prescribing:

Registered practitioners who are Non Medical Prescribers are accountable for their prescribing activities by ensuring they are aware of their professional accountability and any restrictions or special considerations in their prescribing practice. Qualified Non-Medical Prescribers must register their qualification with their Professional Body and with the Trust Database in accordance with the Trust Non-Medical Prescribing procedure. Supplementary prescribers must work within Clinical Management Plans in close association with a medical practitioner.

MANAGERIAL

Undertake delegated responsibilities as agreed with the Team Manager.

Support the formulation and implementation of clinical and operational practices and procedures as appropriate.

Participate in the recruitment, selection and induction of new staff.

Support the implementation, monitoring and updating of an operational policy and philosophy for the clinical area.

Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high quality service based on agreed clinical standards.

Ensure that service users are cared for within the confines of their respective Mental Health Act status and adhere to the requirements of the MHA 1983 and the corresponding Code of Practice.

Ensure that Trust policies and procedures are adhered to. Assist in the development of local protocols to ensure a high standard of service delivery is achieved and maintained.

Support the process of staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.

Ensure effective processes of communication are maintained and regularly evaluated with stakeholders.

Assist in the investigation of complaints and incidents in accordance with Trust policy.

Participate in Serious Untoward Incident investigations/reviews as required.

Ensure that the practice within the clinical area adheres to the Trusts policies and procedures.

Participate in supervision both as a supervisor and supervisee as agreed with your Line Manager.

Participate and support the team in managing changing policies, strategies and procedures on practice. Taking on new work and make changes to own work when agreed, requesting relevant help if needed.

Constructively identifies where new ways of working, polices or strategies are having a negative impact on the service given to users or the public.
