An opportunity has arisen to join the hospital in reach team (HIRT) of Forward Leeds.

The role is excellent chance to deliver high quality addiction interventions in an exciting, fast paced, quick turnover, clinical setting.

The team is well established and has a large amount of clinical credibility within one of the country's largest NHS trusts.

The team cover all LTHT sites and receive referrals from a variety of clinical areas of the trust so the role is an agile and responsive one.

The HIRT bridge the gap between LTHT and Forward leeds.

The shift pattern is 9-5 Monday-Friday. In order to provide the seven day service. The post will include 1 weekend out of 4 (including bank holidays). The work shift pattern is a rolling rota of a 6 day week (when working the weekend) followed by a four day week then two weeks of mon-fri.

Main duties of the job

The primary function of the team is to provide a liaison role between the LTHT, Forward Leeds and services users. This involves communicating any updates, regarding the physical or mental health of services users, to the service user or other relevant people involved to ensure safe delivery of care. The Hospital will also ensure that any treatment initiated by Forward Leeds is continued in hospital and conversely ensure that this continues smoothly on discharge.

The second key role for the Hospital In reach team is to attempt to engage patients admitted to LTHT with drug or alcohol problems to engage/re-engage with Forward Leeds upon discharge. This is an excellent opportunity to hone or adapt motivational interviewing skills.

The hospital environment can provide the practitioner with a good opportunity to deliver addiction interventions such as behavioural analysis, ISBNT and relapse prevention for those services users who are admitted for a prolonged period thus capitalising on an admission to cease or stabilise their drug or alcohol use.

The Final key function is to assist LTHT staff, through advice and education, in managing alcohol or drug withdrawal, safeguarding, or any other issues arising from the services users drug or alcohol use.

The Hospital team work in partnership, and regularly do joint assessments, with a number of different specialities within LTHT such as the hospital mental health teams, ALPS, the pain team, social services and safeguarding.

Job description Job responsibilities

• Liaison with Forward Leeds, LTHT and LYPFT, as well as other professionals and statutory and non-statutory bodies.
• The post holder will be able to demonstrate a high level of skill in conflict management and resolution within and outside of the Hospital in-reach team
• Adhere to Professional, Trust and service specific policies and procedures, including the reporting of incidents and accidents, complaints and the management of confidential information.
• Demonstrate a high level of communication skills both internally within the Trust and externally with other professionals, providers and agencies.
• Attend and contribute to meetings of own professional discipline to discuss contemporary issues and sharing of good practice.

• Be competent and confident, in providing expertise, advice and to LTHT healthcare professionals on the management of patients with co-existing physical health, mental health and substance misuse problems.
• Able to communicate with service users who may be distressed and who may, on occasion, present with challenging behaviour.
• Ability to supervise nursing students on placement.
• Evidence of relevant post registration/qualification.
• RMN and/or RGN with at least three years post registration experience and two years working in the substance misuse field.
• Positive attitude to the client group.
• Evidence of relevant addiction experience in delivering psychosocial interventions
• Be able to demonstrate effective working partnerships within a multi-disciplinary team, to achieve an integrated, effective, and efficient service.

Person Specification Qualifications Essential

• RMN or equivalent professional qualification. Post graduate qualification in Addiction studies or equivalent.

Skills / Experience Essential

• Demonstrates a high level of understanding of the difficulties experienced by people presenting with substance misuse problems and/or mental health and/or physical health needs and how this impacts on function.
• Excellent Communication and Interpersonal skills
• Be able to demonstrate effective working partnerships within a multi-disciplinary team, to achieve an integrated, effective and efficient service.
• Demonstrate a high level of skill and expertise in effective decision making whilst exposed to distressing and highly emotional situations.
