Main duties of the job
Bolton Community Mental Health Services are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties in Bolton 7 days per week, 365 days per year.
The service calls upon a wide range of evidence based holistic assessments and interventions.
As a Community Psychiatric Nurse working in the service you will be required to undertake the role of Care Coordinator in line with CPA process, you will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway, and will be expected to work proactively and flexibly within a multi-disciplinary team model.
There is a requirement to provide clinical leadership and to assist in the organising, supporting, supervision and development of junior staff members.
You will also be expected to act as assessor/mentor for student nurses.
The post holder will play a key role in the further development of the service and in the implementation of its Clinical and Social Care Governance agenda.

Job description Job responsibilities
- To assume the role of care coordinator for a complex caseload.
- To work as a member of a multi-disciplinary team.
- To have a theoretical and working knowledge of the Safeguarding Adults and Childrens framework, and to take the lead role when necessary.
- To undertake initial assessments of people referred into the service, linking in directly with the Team Leader and wider MDT.
- To develop, implement and evaluate holistic programmes of care, including the assessment of carers needs and assessments.
- To carry out clinical and therapeutic interventions, having regard to both legal requirements and Trust/National guidelines.
- To supervise, support and develop junior staff members and act as student assessor/mentor when required.
- To make a positive contribution to the development of mental health and social care services in Bolton.
- To maintain close links with other statutory and non-statutory professionals and agencies, and to be able to access resources from provider services, and to maintain good channels of communication.
- To undertake the role of care coordinator in accordance with the philosophy of the Care Programme Approach and associated Mental Health Act (1983) legislation and Care Act legislation.
- To participate in the office Duty System as required, and be able to respond to emergencies.
- To work with people who are on the Assertive Outreach care pathway; meaning that you will be expected to work outside normal hours of working.
- Act in accordance with practice placement standards for student nurses, liaising with link tutors from the University in the evaluation and validation of students.
