Post Title Kirklees Home Based Treatment Team37.5 hours per week
Band 6 AFC
Kirklees Intensive Home Based Treatment Team are looking to recruit an enthusiastic individual to join our existing team as a Band 6 practitioner.
The IHBT provides intensive home based treatment 7 days a week, 24 hour working as an alternative to hospital admission.
The post holder will be responsible for the triage of referrals and be responsible for assessments, care-planning and co-ordination of care delivery within a team approach.
You should be forward thinking, proactive and it is essential that you are willing to work flexibly as part of a 24 hour service, which includes shifts and working weekends. The role provides the opportunity to work across the health & social care spectrum as part of an integrated team which has links to a multi-disciplinary service. The ability to form effective professional working relationships with your team colleagues and service users is an essential requirement of the job.
Applicants should be registered mental health practitioner (RMN, Social Worker, Occupational Therapist) with experience at Band 5 or above in a mental health service for adults and meet the essential requirements for the post in the person specification. Car driver is essential to this post. A period of induction and preceptorship will be available for the successful candidate.
