The OT team are an integral part of the Specialist Dementia and Frailty Service within the beautiful West Essex locality. We deliver an assessment, diagnostic and treatment pathway for patients suffering from memory problems across all age spectrums as well as complex case management for patients experiencing functional mental illnesses who are over the age of 75 who have an age related frailty.

You will be supported through strong professional leadership, career development opportunities and access to ongoing training, CPD and clinical supervision. Our links with local Universities present a variety of opportunities for OT staff to take an active part in student education and training.

Main duties of the job

  • You will undertake the comprehensive assessment of patients needs in the West Essex area, using relevant occupational therapy standardised and non standardised assessments and outcome measures.
  • You will be developing, facilitating and reviewing, occupational therapy groups such as Cognitive Stimulation Therapy, in line with the evidence base.
  • Assessing and treating your own specialist community caseload of patients in accordance with occupational therapy evidence based;using goal setting and interventions focused on re-engagement in self-care, productivity and leisure activities, and promoting health, wellbeing and independence.
  • You will provide occupational therapy advice to other disciplines and be involved in providing training, supervision and support to others as required. You may lead clinical audit and undertake research.
  • You will provide leadership in the delivery of specialist occupational therapy interventions, and provide care coordination to caseloads have responsibility for the day to day supervision and management of lower banded staff within your professional group and team locality.
  • You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services.

We deliver an assessment, diagnostic and treatment pathway for patients suffering from memory problems across all age spectrums as well as complex case management for patients experiencing functional mental illnesses who are over the age of 75 who have an age related frailty.

The SDFS is a multidisciplinary team made up of OT's, CPN's, Phycologist, Doctors, Support Workers and Admin; who all work together to provide the client centred care our patients require to best meet their needs. Job description Job responsibilities

Clinical Function
  • Responsible for the initial assessment process, as well as observation and the on-going assessment, providing accurate and timely feedback
  • Responsible for the planning, implementation / evaluation and on-going review of treatment plans.
  • Provide formulation, delivery and review of individualised evidence based care / care packages according to NICE guidance including non-medication strategies.
  • Organise and deliver therapeutic activities for service users.
  • Assess service users response to activities in accordance with agreed models of practice e.g. MOHO
  • Provide practical support, advice and education to service users, carers and other disciplines.
  • Provide specialist OT advice on equipment, telecare etc. to other disciplines.
  • Assess service users for access and allocation of equipment within their individual needs.
  • Responsible for the recording of all patient related activity information onto System One.
  • Lead induction of staff and fieldwork education of students.
  • Produce accurate and timely documentation on all clients in line with your Professional Code of Conduct and Trust policies
  • Carry out risk assessments including any potential hazards in the service users environment.
  • Attend multi-disciplinary meetings i.e. referral / review / business.
  • Report and document incidents as required.
  • Lead and facilitate group work such as Cognitive Stimulation Therapy, health promotion etc.
  • Undertake carers assessments in line with Trusts key performance indicators.
  • Be competent in use of relevant screening and assessment tools.
  • Provide comprehensive assessment, care planning, treatment and review of care in line with the requirements of Mental Health Care Clusters.
  • Participate in the continuing development of a high quality service that is underpinned by evidence based practice, care pathways / packages, NICE and DH guidance.
  • Participate in audit and research where appropriate and contribute to the development of service policies.

Non-Clinical Function
  • Promote Occupational Therapy within the service and wider community.
  • Participate in the implementation of the OT Strategy.
  • Take part in service development and review.
  • Identify aspects of the service which need to be improved and implement appropriate changes in conjunction with the Clinical Manager, and Advanced OT Practitioner and Area Consultant OT, where appropriate.
  • Participate in the recruitment process of OT and other clinical staff as required.
  • Provide clinical and managerial leadership and expertise to team.
  • Ensure the service meets the agreed performance / contractual targets and adheres to local and government initiatives.
  • Work closely with and assist Clinical Manager / professional lead with development and implementation of team operational policies.
  • Deliver training where appropriate for other professionals.
  • Manage the OT capability process as appropriate.



ADDITIONAL DUTIES


  • Complete mandatory training in line with Trust policy and procedures
  • To keep yourself updated on all matters relating to Trust policy
  • You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required.


Person Specification Education/Qualifications Essential
  • Diploma / Degree (Bsc / Msc) in Occupational Therapy
  • HCPC Registered
Desirable
  • Experience in developing and facilitate therapy groups
Education Essential
  • High standard of education Good standard of written and spoken English
Skills/Experience Essential
  • Proven experience of working in a health, social care or community setting as part of a multi-disciplinary team
Desirable
  • Proven experience of working with people with dementia and mental health conditions and vulnerable service users
Additional Qualities Essential
  • Car driver with current driving licence
