Working in partnership with primary care and third sector organisations to promote early diagnosis of dementia, provide post diagnostic information and support to service users and their carers and signpost those not suitable for treatment, according to agreed shared care pathways.

Main duties of the job

To work in the community within a memory assessment serviceTo provide initial assessment with patients presenting with a cognitive declineTo manage a caseload of clients with complex and challenging mental health needsTo assess clients in the most appropriate location often in their own homeworking in accordance with the Lone Working PolicyTo plan and implement client centred interventions using selected and graded activity to meet the needs of the individual ensuring issues of behavioural disturbance, aggression, wandering and mood disorder are consideredContribute to making decisions regarding client care in consultation with the multi-disciplinary team.Undertake the assessment, planning, implementation and evaluation of specific packages of care (in conjunction with other team members).
