The Bridport Community Mental Health Team are seeking to recruit a registered Mental Health Nurse to join our small, friendly, well-established team. We are looking for an innovative, creative, motivated, and caring individual who has experience working with people diagnosed with a Serious Mental Illness (SMI) whether this be in an inpatient setting or in the community.

The primary role will be to act as Care Coordinator for a dedicated caseload of patients with whom you would provide person centred, evidence-based, targeted interventions to aid the clients’ recovery. There will be opportunities for you to advance you own clinical assessment skills whilst risk assessment will remain at the forefront of everything you do. Alongside your caseload work, you will be rota’d to cover our Duty system and support with our Depot clinic. You will have a working knowledge of current models of care and apply this understanding of working with people with an SMI in your attempts to engage and promote recovery in your work.

You will work closely with our Integrated Multi Disciplinary Team that consists of Consultant Psychiatrists, Mental Health Nurses, an Occupational Therapist, Social Workers, an Advanced Nurse Practitioner, Support Time Recovery Workers, a Physical Health Worker and our Psychology Team.

We believe in working collaboratively and supporting one another in challenging situations. You will be provided with regular clinical supervision and we will support your career development. If you are thinking of applying from a Band 5 position, you will be given support to develop into your role.

The successful candidate will have a proven ability to provide comprehensive psychosocial assessments, risk assessments, formulations and provide evidence-based interventions for adults of working age with an SMI.

The team is based at Bridport Community Hospital and covers Bridport, Lyme Regis and surrounding areas and as such you will need to have a valid driving licence and the use of a car as you will be required to review and monitor patients in their own homes and within other community settings.

This post has become available at a very exciting time. Significant investment has been made available to review and reconfigure community mental health services and this is taking place right now via the Mental Health Integrated Community Care Project (MMHICC). Within this post you will have the opportunity to participate in the project shaping our future services.

This post is full-time, 37.5 hours per week but part-time and flexible working will be considered.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
