An exciting opportunity for the post of Band 6 Social Worker has become available within East Hertfordshire Adult Community Mental Health Services.
You will have experience of working in a community based setting. You will be responsible for the delivery of care to an allocated caseload of clients living in the community experiencing, or recovering from, mental health difficulties.
You will provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control. You will ensure that appropriate health and social care services are commissioned as required. You will also assess, identify and evaluate Client’s needs through comprehensive assessments, reviews, care planning and CPA in conjunction with other professionals.
You will work as part of a Multi-Disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust.
The post holder will be required to demonstrate their knowledge of the relevant legislation, theory and policy in relation to the care group, such as the Care Act and The Mental Health Act.

A ‘Golden Hello’ payment of £3,500 to new employees. The payments will be non-pensionable and will be subject to tax and national insurance deductions.
