We are looking for a Band 6 Physiotherapist with an interest and experience in stroke rehabilitation to work collaboratively within the community stroke service across the Scarborough and Ryedale locality.

You will be expected to hold a clinical caseload within the community stroke team and support with provision of a seamless stroke service from the local acute hospitals into the community.

We are looking for someone who is enthusiastic, conscientious and adaptable. The successful candidate will have excellent communication skills and be able to demonstrate an adaptable approach whilst meeting guidelines and stroke standards. You will be confident to manage your own caseload, without direct supervision, as you will be responsible for delivering highly specialist assessments and treatment. We expect that you will provide support and clinical supervision to less experienced colleagues. You will have high levels of clinical knowledge, and excellent planning and organisational skills as you will play a crucial role in the ongoing review of the service.

Main duties of the job

We provide assessment and rehabilitation within the community setting for adults, from 18 years upwards who have had a Stroke. We work closely with professionals, services and agencies across the whole Stroke pathway.

You would be part of a small, dedicated, knowledgeable multi-disciplinary stroke team. You will have regular supervision and training opportunities. You will be joining the team at an exciting time of service development towards implementing the National Service Model for an Integrated Community Stroke Service, with a focus on creating a robust early supported stroke discharge service. You will have the opportunity to develop clinical skills and knowledge, operational awareness, service improvement and leadership skills.

Location

Although the base for the Community Stroke Team is at Prospect Road in Scarborough, the team have adapted our way of working through the COVID pandemic and have the infrastructure and processes in place for remote working. We complete MDT, team meetings and training via Microsoft Teams. As a team we have regular communication via MS Teams chat, video calls and phone calls. We complete patient records and documentation remotely using SystmOne.

Person Specification Knowledge Essential

  • Evidence of promoting/supporting active patient/relative involvement/participation

  • Ability to effectively commute between the organisations various sites
  • Occasional to moderate sitting in a restricted position, manoeuvring therapy equipment and or patients, active physical movement

  • Post graduate experience in Stroke / Neurology
