Oxford Health Foundation Trust are pleased to announce that there is a Band 6 Occupational Therapist vacancy for our new Community Mental Health Rehabilitation Team in Buckinghamshire. The post holder will work as part of multi-skilled team to deliver a recovery focussed community rehabilitation service for individuals with severe and enduring mental health (SMI).
As the Occupational Therapists in this role, you will work with individuals under the rehabilitation team to enable them to live independently as possible and manage their health and well-being needs. You will hold a varied caseload, offering time limited, evidence based Occupational Therapy assessments and interventions with a recovery and rehabilitation focus. The postholder will work collaboratively with staff and individuals to co-produce and develop support plans and SMART goals that are tailored to individual needs and interests that aid recovery and develop independence.
The rehabilitation team Occupational Therapists will work in conjunction with inpatient/ CRHTT and AMHT Occupational Therapists to deliver seamless transition of interventions between teams where residual/continued assessment or intervention is required.
