We have a vacancy for a qualified practitioner in our older adult CMHT in Spalding. This would suit an experienced registered mental health nurse/social worker/OT, however we would be happy to receive and consider applications from a variety of qualified healthcare practitioners including registered general nurses, paramedics etc. We would also encourage a Band 5 practitioner to consider applying for the post. We will provide the structure and training to enable the successful candidate to grow professionally to meet the competencies of a Band 6 role. The post is full time, Monday to Friday 9-5

The team supports patients over the age of 65 who present with serious mental illness. We also provide an all-age memory assessment and monitoring service.

The team consists of a Consultant Psychiatrist, a specialist doctor, senior mental health practitioners, Associate practitioners, and a dementia support service. The older adult division encompasses CMHT’s inpatient wards and home treatment teams as an alternative to hospital admission.

As a division we work closely together on a joined up patient pathway to ensure transitions between services are smooth and aid patient recovery.

The successful applicant will be providing high quality person-centred care to patients, families and carers. You will act as care co-ordinator/Lead HCP, undertaking comprehensive evidence-based assessments, supporting patients to work towards recovery goals, build self-resilience and promote independent living. You will be caring for and coordinating for a caseload of patients who are on both the mental health and the memory assessment pathway.

Our memory pathway supports those patients referred in for a memory assessment. Your role will be to undertake a comprehensive assessment as provided within our pathway and support the patient and their family/carers through diagnosis and treatment, and on to appropriate services as applicable. Training and support for this will be provided if this has not previously been part of your job role.

Spalding adult and older adult CMHT’s are in the second wave of the Community Transformation roll out. Local Integrated Place Based teams are being developed to support those patients with serious mental illness who do not need to be in receipt of secondary services. The transformation pathway is developing a step up, step down approach to ensure a smooth passage for patients through services at differing times of need, with a “no wrong door” philosophy

You will also be working alongside Neighbourhood Teams, Adult Social Care, GP surgeries and other partner organisations to ensure the best outcomes for patients and carers.

The team is based at The Johnson Community Hospital in Spalding. This is a modern purpose-built building on the outskirts of Spalding with easy access to the surrounding road networks. There is ample free parking at the site.
