Enhanced Care Service/Intensive Support Team – LD & Autism
Based in Hereford
Learning Disability Caseworker (RMN/RNLD/OT/SW)
Band 6
37.5 hours per week – Mon to Fri 9-5
The Intensive Support Team is all about improving health and care services so that more people, who have a learning disability and/or autism with mental health needs and forensic needs, can live in the community, with the right support, close to home. This supports the successful implementation of ‘Building the Right Support.’
We are on the brink of some service change in this small countywide team and have received investment to expand our workforce. The addition of this post holder to the team will enable us to better meet the needs of our clients who may be at risk of their care breaking down
The successful candidate will specialise in the complex assessment, management and treatment of needs for those who have Learning Disabilities and or autism, offering bespoke pathways to recovery within the community.
We are looking for a keen team player who has a good understanding of the transforming care agenda. The candidate should have knowledge around person centred care which is reflected within positive behavioural plans, risk assessments/risk management plans and relapse plans that incorporate low arousal approaches and promote least restrictive practice.
The successful candidate will ideally have experience and knowledge of mental health need amongst people with a complexity of LD/A related needs and challenges. The applicant should be able to work flexibly and with minimal supervision with the ability to cope with changing routine. A strong understanding of mental health, mental capacity and criminal justice legislation and frameworks is desired.
We can offer you a small friendly team, flexible and family friendly working alongside training in specialist risk assessment relevant to this new role. We will also ensure the post holder receives suitable clinical supervision and is supported in role by the wider community learning disability team.
The post is a full time contract of 37.5 hours a week. The post holder will require a flexible approach to assist when and where required between the hours of 9am to 5pm Monday to Friday and occasionally later where agreed.
The team is based at Commercial Road Hereford and a flexible mixed working from home/office pattern will be agreed .
