An exciting opportunity has arisen in Supported Care for an Occupational Therapist to join the Supported Care (North) Team in delivering high quality therapy interventions to patients in their own homes. Supported Care provides effective short term reablement/rehabilitation programmes for vulnerable patients with acute health needs. This position would be ideal for a professional looking to work in the community in an evolving integrated team.

Main duties of the job

The Supported Care Service is commissioned to provide an MDT approach to prevent unnecessary hospital admissions and to support timely discharges from both acute and community hospitals.

Key Areas of Responsibility

• To work within the supported care team to prevent unnecessary admission to hospital.
• To make risk assessments, establishing the safety or otherwise of a patient's ability to stay at home, or return safely from another care setting, providing and arranging as necessary supportive packages of care.
• To provide evidenced based clinical/therapeutic interventions based on 'best practice' in order to improve health outcomes and promote choice.
• To provide in-reach to inpatient units and step up beds where required
• To act as the patients advocate, facilitating choice, patient empowerment and independence.
• To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

About us

The Supported Care Service is a seven day service commissioned to provide an MDT approach to prevent unnecessary hospital admissions and to support timely discharges from acute and community hospitals. The team consists of Physiotherapists, Occupational Therapists, Nurses, Palliative Care Specialists and Assistant Practitioners, working closely with Norfolk First Support as part of the Network of Escalation Avoidance Teams (NEAT) and Discharge to Assess facilitation pathways.

The post holder will deliver high quality Occupational Therapy / Physiotherapy interventions to patients in their own homes, and provide short term reablement/ rehabilitation programmes with effective short term goal setting for vulnerable adults with acute health needs. This will involve using health coaching skills to effectively engage with the patient and their family to ensure expectations are appropriately managed.

The post holder will demonstrate a flexible approach, being able to adapt and respond effectively and efficiently in potentially stressful case scenarios. The post holder will use their Physiotherapy diagnostic skills within their assessment to determine need for further investigations, and to support the MDT with ongoing patient management.

The post holder will show a level of resilience by working with the team to develop competencies in all areas of practice. Each clinician within the service has developed competencies in Physiotherapy, Occupational Therapy and Nursing. It would be an expectation for the post holder to be involved in the training of other team members to maintain competence within some aspects of Physiotherapy, as well as develop and maintain own competencies in areas of Occupational Therapy and Nursing.

• Diploma or 1st level degree in Occupational Therapy / Physiotherapy
• Current HCPC registration
