An exciting opportunity has arisen for experienced, highly motivated and dynamic individuals to join our Aneurin Bevan University Health Board Transfer Practitioner Team. As a Transfer Practitioner you will play a vital role in the safe and timely transfer of unwell or deteriorating patients or those requiring time critical interventions from hospitals within the Health Board to the Grange University Hospital or to a specialist tertiary hospital.
Applications are sought from nurses and ODPs who have the appropriate expertise, knowledge and experience to undertake the role enabling them to assess and manage unwell or deteriorating patients during the transfer/retrieval phase, while striving to deliver high-quality, safe and effective care.
Successful applicants will work 50% of their hours within a Transfer Practitioner role and the other 50% in Critical Care or in a clinical area to be agreed on appointment depending on background and experience.

The Grange University Hospital (GUH) is a new Specialist and Critical Centre within the Aneurin Bevan University Health Board (ABUHB) and forms part of the ABUHB Clinical Futures model. GUH provides a centre of excellence to treat the most seriously ill patients or those with significant injuries. The hospital has 560 beds and features an Acute Assessment Unit, Emergency Department and Helicopter Pad, providing a 24/7 emergency service for patients that are seriously ill or have complex problems or conditions that cannot safely be managed in one of the Local General Hospitals. The patient flow between the Local General Hospitals and GUH is coordinated by the ABUHB Flow Centre. The Flow Centre combines two central functions, pre-hospital streaming, and the co-ordination of inter-site transfers. The Transfer Practitioner plays a part in the inter-hospital transport model, ensuring that deteriorating patients can be transferred to the correct hospital in a safe and timely manner.
The primary role of the Transfer Practitioner is to safely and in a timely manner, transfer patients ward to ward Level 1 and Level 2 patients agreed following consultant to consultant clinical decision making, as being 'deteriorating and requiring a timely transfer to a specialist/higher acute setting'. This cohort of patients will also include some tertiary transfers and may include paediatric patients.About us
