The post holder will work as a member of the Continuing Healthcare registered nurse team to assess, plan, implement and evaluate care under the direction of the Case manager. To monitor competency levels of healthcare support workers to ensure the provision of effective, efficient, appropriate quality care to patients with complex needs in the community.

We are looking for nurses who have the drive to think outside the box, to deliver tailor made care to enable individuals with multiple holistic care needs to maintain fulfilling lives, be treated with dignity and respect and remain in their own home.
You would be required to use your own initiative and have the ability to manage your own workload effectively whilst being supported by a team of other band 5, 6 & 7 nurses within your locality team.
The role would also involve liaising across the multidisciplinary boundaries and who always place the patient at the centre, developing skills within continuing health care assessment and reviewing, risk management - to name a few.
Main duties of the job
A secondary yet equally important part of your role would be the managerial and staff support role in which you would be working with a team of highly skilled Health Care Support Workers and managing their clinical competencies to enable them to continue to deliver care to the clients within your caseload.
This may involve development of your own clinical portfolio for which support will be given by the wider team and may also enable you to develop teaching skills and develop mentorship roles.
This role is both challenging and rewarding. In return, we can offer you a career in the Care at Home Team with support to develop your skills at every level. We have support from a dedicated educationalist to plan, develop and deliver training tailored to the needs of the service and you as an individual clinician.
Clinical skills range depend on the holistic needs of the clients we care for but can range from psychological needs, mobility care, complex bowel care , airway management with more specific skills including tracheotomy care and airway management and mechanical ventilation. Dietetic rigg care -changing etc. Comprehensive training will be given to deliver this care in a community setting.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
