We are looking for a Mental Health Practitioner (MHP) to join the Community Forensic team based at Easby House, in Dawlish Devon.

This is an exciting opportunity to join our newly expanded community forensic team at one of the country’s most respected providers of secure mental health services. The community forensic Team have now been shortlisted twice for the Nursing Times wards in 2020 for team of the year and in 2021 for community mental health team. We are also winners with national positive practice in mental health awards 2021 winning innovation in community mental health services award and being highly commended for Covid team of the year.

The role is dynamic and challenging. The overarching aim of the Community Forensic service is to deliver a model of integrated care and support across all elements of a service user’s pathway. With a specific focus on prevention through admission avoidance and early intervention through collaboration with other services. This includes training and support of staff, case consultation and formulation. For those in inpatient care there is an aim to support flow through the secure directorate and reduce re admission following discharge into the community. The team works with a relational approach with a focus on integration, meaningful roles and routines in the community.
About you
As a SeniorMental health practitioner within the Community Forensic Service, you will be a core member of a multi-disciplinary team and be responsible for taking an active role in carrying out the functions of that team; including carrying a case load of varying complexity. You will work in partnership with another Care coordinator to provide holistic client centred care co-ordination. You will have a key role in our recovery pathway, providing direct nursing care and also engaging in a range of therapeutic interventions.
Secure Services
We have 2 Local Delivery Units (LDUs) at various locations across South Devon including our Inpatient, Community Forensic and Criminal Justice Services as well as our Prison Mental Health Teams.

The Directorate supports people who, as a result of their mental health issues, have had contact with the legal system and need a safe and secure environment that enables them to receive a wide range of treatments, therapies and care to help them recover. This includes contact within Liaison and Diversion services; assessment and treatment within our 3 Devon prisons; secure inpatient care and discharge into the community.
