We are looking for a highly skilled and compassionate Registered Nurse to join our Matron Team.

The successful candidate will work with Central Neighbourhood Team to support the residents in this area. This is a permanent full time position, part time will be considered.

It is the expectation that the successful applicant has achieved non medical prescriber V300 certificate.

Main duties of the job

To support residents in their own home who are acutely unwell , providing same day response to enable patients to stay at home and prevent hospital admissions where safe to do so. There is potential for case management where you will have responsibility for planning, managing and coordinating the care of those with highly complex needs and long term conditions .

A primary objective will be to work collaboratively with patients in their own homes, the acute hospital and community teams to ensure that residents receive the right care, at the right time in the right place.

We are looking for someone with excellent leadership, organisational and communication skills, and a strong quality improvement focus- as well as the ability to analyse and interpret data.

Job responsibilities

  • A highly skilled senior member of the integrated Neighbourhood team supporting patients within their own homes.
  • The post holder will have a clinical role with responsibility for same day assessments using extended skills and independent prescribing to support patients at home. Planning, managing and co-ordinating the care of patients under your care.

Person Specification



  • First level Registered Nurse with current registration with the NMC
  • Prescribing/ability to work within PGDs
  • Specialist Diploma level/ specialist practitioner or equivalent
  • Evidence of CPD in management of LTC and clinical investigation skills
  • Leadership training (LEO/LPC) or equivalent
  • Independent Nurse Prescribing course


  • Degree in health related topic
  • Teaching qualification
  • Advanced communication module
  • Ability to be able to complete DNARCPR when required.



  • Evidence of extensive expert professional/ clinical knowledge in Care Homes and Frameworks.
  • Effective communicator able to communicate complex and highly sensitive information
  • Evidence of effective people management and leadership skills
  • Evidence of managing staff and multidisciplinary teams
  • Evidence of relevant involvement in meeting the Trust clinical governance objectives
  • Be able to demonstrate the skill to integrate teams from a variety of backgrounds to integrate the social and health agenda
